How to Establish a Palliative Care Program

Operations

How will operations be determined and reviewed?

Standards of Practice

What norms of practice will you use to guide development of your program's standards of practice? Who will develop them? What process will be used to develop the program's standards of practice? Will it be "top-down" or by consensus?

Policies and procedures

Specify the clinical practice guidelines that will be used to guide policy and procedure development.

Measurement tools

Specify the measurement tools that will be used to assess patients and collect data about the effectiveness of your program

Collaboration

Define those entities within the host organization with which the palliative care program will collaborate. For example, some palliative care programs work closely with anesthesia pain services, ethics consultation services, cancer centers, comprehensive geriatric programs, etc.

Continuity

Determine how will the program will facilitate continuity of care across settings of care. This consideration is particularly important when the palliative care program won't provide palliative care services in all possible patient care settings.

Frequently, achieving the goal for providing continuity of care will necessitate collaborative relationships with several community resources.

Sequence of Service Development

The palliative care program is likely to include several elements such as an educational program, a hospice organizational partner, a consultation service, an inpatient unit and a home (outpatient) component. Decide in what sequence you will develop and implement each of these.

Programs rarely implement all service components simultaneously. They build over time. An early partnership with a community hospice organization can "jump start" the delivery of palliative care across settings.
